This Notice applies to all Protected Health Information (“PHI”) created, received, or maintained by the Continuum GUIDE Program (“we,” “us,” or “our”).

Continuum GUIDE Program is committed to protecting the privacy and security of your health information. We are required by law to:

- Follow the terms of this Notice currently in effect

By checking a consent box, submitting your information, or otherwise opting in, you consent to receive text messages related to your services with the Continuum GUIDE Program.

Opt‑Out
You may opt out of receiving text messages at any time by replying STOP to any message or by contacting us directly. Opting out of text messaging will not affect your ability to receive services.
E. No Selling or Sharing of Personal Data
Continuum GUIDE Program does not sell, rent, or share patient phone numbers or personal information with third parties for marketing purposes.
Your information is shared only as permitted or required by HIPAA and applicable law, including with trusted service providers who assist us in operating the program and who are required to safeguard your information.

By consenting to electronic communication, you acknowledge and accept these risks. Whenever possible, we encourage the use of secure messaging platforms.

Continuum GUIDE Program may use third‑party vendors (such as OhMD) to facilitate secure communications, data management, or operational support.
All vendors with access to PHI are required to:
