Understanding the Medicare GUIDE Program

Caring for a loved one with dementia is both a rewarding and challenging time, and as professional caregivers, we know how important it is for families to have support. That’s why we are proud to be a provider of services through Medicare’s GUIDE program.

To be eligible for Medicare’s GUIDE Program a beneficiary must meet the following criteria:

  • Be a Traditional Medicare beneficiary
  • Have a confirmed dementia diagnosis, such as Alzheimer’s, LBD, FTD, or vascular dementia
  • Be enrolled in Medicare Parts A and B, with Medicare as their primary payer
  • Not be enrolled in Medicare Advantage, including Special Needs Plans, or PACE programs
  • Not have elected the Medicare hospice benefit
  • Not be a long-term nursing home resident

What are the benefits of the GUIDE program for Medicare Beneficiaries?

GUIDE offers significant benefits by improving access to comprehensive care. Through a structured model, GUIDE integrates home visits, caregiver education, and support to ensure personalized care, including respite care services in your home, adult day program services, and 24/7 community care.  Additionally, the program provides Medical Oversight with clinical recommendations and ongoing care management led by a multidisciplinary team. We collaborate closely with your existing healthcare providers to seamlessly coordinate and enhance the quality of your care, making sure that all aspects of your health are effectively managed.

Will additional documentation or referrals be required?

Yes, additional documentation or referrals may be required for patients to enroll in the GUIDE program. Healthcare providers will need your Medicare cards and some medical records or information that confirms a patient’s diagnosis of dementia or cognitive impairment. In some cases, a formal referral from the patient’s primary care physician or specialist may also be necessary to initiate participation in the program.

Am I eligible for GUIDE?

Eligibility for Medicare’s GUIDE program focuses on individuals diagnosed with dementia or cognitive impairments, along with their caregivers. Beneficiaries must be enrolled in traditional Medicare and meet specific medical criteria to qualify for the program’s services. How is caregiver education delivered?

Caregiver education in the GUIDE program is delivered through a combination of in-person and virtual support sessions. Additionally, the program provides ongoing care navigation calls, education through online resources, workshops, or support groups to help caregivers stay informed and connected.

How much does GUIDE cost?

Services provided under the GUIDE program are covered by traditional Medicare at no additional cost to you.
