Specific programs are tailored for each community to complement the market niche of the building and the needs of its residents. 

All programs are managed by our nurses and social workers and include private duty care, supportive service visits based on tasks, and full service bundled programs designed to meet the 24 hour needs of residents.

    Bundled Programs: Services Available to All in a Designated Wing or the Whole Community
    Continuum’s staff is available 24/7 to meet the needs of the participants in a bundled service program.  A lump sum monthly fee allows residents to benefit from this program.
